Interesting Dog Tags

Saw these tags on an early WWII Dog Tag Chain. Interesting that the guy has his address and phone number instead of service number and service-related information. Are these issue or is this someone's fantasy or use during Civil Defense? If anyone's seen these before and can comment I'd appreciate it.

My guess is that if these are period, that they were made for a relative or friend. Perhaps one of the GI's at a base or induction center made them or had them made by a friend.

Those supply sergeants can get almost anything. :lol:

When I was a kid (many, many years ago) I had a set of dog tags like these that you could send away for. I believe that is was a promotion on a cereal box or something like that.
