That's really interesting. First I've seen. I would say it signifies 18 months overseas in WW1 with the 26th Div - worn on the lower left sleeve. Probably an unauthorized item worn by some state unit in the 1920's.

I concur with Kurts observation and go one further it could have been a veterans organization thing.

It is a patch worn on the uniform of a member of the YDVA (Yankee Division Veterans Association) This group was very active in the Boston area during the 1920's thru the 1950's.

 

Their headgear was a black beret with a yellow YD embroidered on it.

 

The Association medal is very scarce.
