"Eagle Flight" friendly fire casualty

CW2 John W. Garrett, US Army Blackhawk pilot, was one of numerous casualties on 14 Apr 94 over Iraq - when shot down by F-15s during Operation Provide Comfort.

 

In attempting to complete his basic biography we've not been able to locate:

 

- Date and Place of birth

 

Here is what we have thusfar:

We know he had been enlisted in the USAF, and had joined the Army by the time of his death.

 

Can anyone help us get that information please?

They did have him listed there, and a photo, and the correct patch - but still no place and date of birth.

 

I've searched everywhere I know of.
