The number is just 764 no pre fix.  This is an Army Spanish campaign medal.

Straight numbers without No. prefix are not traceable for the Spanish Campaign - Typically these are "Contract" medals from the 1920's and 1930's.

 

The manufacturer was either Joseph Mayer Company or Whitehead & Hoag Company.

 

The Spanish War Service (green ribbon) are all numbered without the No. prefix and are traceable up to 16000
