I'm trying to find information about this tag; origin, history, etc. Online search turns up nothing useful.

Any info appreciated.

Thanks,  Zigy

That is a British army form. You may have more luck posting it on our sister site, World Militaria Forum.

US Army forms all have a "DA Form" number, yours just says "Army Form", but the biggest give away is in the 2nd picture where is says OFFICER and OTHER RANK. The first column says British, the 2nd and 3rd say Allied, and Other.

Try an internet search for British Army Form 3004 and you will also find a little information on it. There is also a military store in GB that has them listed for sale.
