RWW Posted May 30 Share #4 Posted May 30 US Army forms all have a "DA Form" number, yours just says "Army Form", but the biggest give away is in the 2nd picture where is says OFFICER and OTHER RANK. The first column says British, the 2nd and 3rd say Allied, and Other. Try an internet search for British Army Form 3004 and you will also find a little information on it.
