WWII US Navy Sailing Location Codes... Any Codebreakers Around Here?

Hello,

I'm trying to figure out (without any luck) what Navy No. 128 and Navy No. 3011 and 3279 mean on the attached form.

128 Pearl Harbor, Oahu, T.H. --> TO 3011 Ulithi Island, Caroline Islands & 3279 Gaja Shima, Japan (Tokara Islands, a sub-group of the Satsunan Islands)
