AZNation, thank you for your work; much more than I was able to dig out with my paltry skills.

This answered the question whether he was an MD, as some MC officers are not.  I notice he had the Bronze Star Medal, not sure where he earned that.  He had quite a career.

 

As for the chair, I am assuming it was a private purchase item, as I don't know of anything like it in the American Army, although it seems somewhat common in the British Army.  As for assembly and disassembly, once put together it breaks down quickly.  Put it up the first time took some thought, but once you put it up once or twice, it seems it could go together fairly quickly as well.
