Questions: WWII USNR Dog Tag & Theater Made Ribbon Bars

Found this group from the same seller.  No history known.  I have a bunch of questions?

1.  When was this style dog tag discontinued?

2.  Why was there no Good Conduct Ribbon?  Assuming he never messed up.

3.  Where these ribbons made in Asia? Philippines?

 

I found these together but I realize that doesn't mean they belong together.

The ribbon with the anchor is the Vietnamese Navy Gallantry Cross.  The most likely reason for no Good Conduct medal ribbon is he was an officer his entire career.

Thanks unclegrumpy.  I just found out, thanks to aznation, he retired as a USN Commander.  He served in WWII, Korea and Vietnam.  Unfortunately, he died in 1985.  He was only 63 years old.  The ribbons definitely belonged to him.
