Wow that is a high sale price! What is the significance is it because of the Porto part ? Or is he a famous?

The small-lettered PR Occupation medals are the rarest of the numbered/attributable campaign medals.

A few reasons: There were roughly 3000 Porto Rico medals issued- a significantly less number compared to its Cuban Occupation counterpart which was roughly 13,000. In addition, only the first 320 struck are traceable. The one Adam posted from the auction is a first strike Porto Rico. It has noticeably smaller font on the obverse and an unprefixed number on the rim. Unprefixed Numbers are traceable up to #200. No. 201-320 are traceable, but carry second strike characteristics, most notably a taller narrower font on the obverse. After No. 320, all others are M No. and have second strike features.
