An Unusual Mothers of the Defenders of Liberty Medal -- NOK?

Some time ago I posted a Paterson, NJ WWI service medal that was unusual because of the attachment of a small gold star to the medal. I surmised it might be a home made next-of-kin medal since the medal Paterson, NJ presented did not use a ribbon like most of the other local medal and thus a gold star of next-of-kin of personnel who died in the service could not be affixed to it.

Here is a link that thread -- http://www.usmilitariaforum.com/forums/index.php?/topic/237875-i-took-a-chance-on-ebay-what-do-you-think/

A couple of days ago I picked something off eBay quite similar. Below are pictures of two Mothers of the Defenders of Liberty medals for the city of Toledo, Ohio. Both have two stars on the top bar indicating the wearer had 2 "sons in service". The one on the left is the standard version that I have had in my collection for a number of years. The one on the right is the one I just picked up. It also has two stars...but wait

One of the stars on the left medal looks different. And it is. It appears to be a gold stat that has been mounted on top of the one that is struck into the bar. It is larger than the other star and stands out quite prominently.

Here are a couple of more pictures.

Here's the back -- looks like all the others from Toledo.

Any thoughts on this one?

Thanks.

Rob, I do not know of a complete list on the forum.  I'm not sure there is a complete list in any event.  Here is a list of the ones I have seen but there are probably others:

Sacramento, CA
Middletown, CT
Albany, NY
Albany County, NY
Buffalo, NY
Canandaigua, NY
Erie County, NY
Fort Plain, NY
Middletown, NY
Rochester, NY
Erie, PA
Johnston, PA
Whitaker Borough, PA
Houston, TX
Martin’s Ferry, OH
Toledo, OH

I have also seen a "Mothers of  men in Service" medal from Metuchen, NJ
