Correct insignia/name patch for USAF orange flight suit

Looking to accoutre my 1960 era USAF orange flight suit with the correct shoulder rank, command patch & leather name patch. Looking to outfit two flight suits: 1. Lt. Colonel w/ command pilot wings & 2. Colonel w/ senior flight surgeon wings. My recollection (as a young boy) is that the rank was a flexible metallic emblem embedded in clear flexible plastic & the name patch was black leather with name & wings only.

 

I'd like to secure the above & would love to insure that they are correct.
