The insignia on the original card is similar to a CPO device but I cannot positively identify it.

Top to bottom - left to right:

Unknown insignia - Officer's Cap Device and Band

Lieutenant Commander Shoulder Marks - Metal Officer's Cap Device - Warrant Officer's Shoulder Marks (Gunner)

That is a CPO's collar rank device and the badge on the band is a Warrant Officer's hat badge.

Lee, I think that is a CPO hat badge rather than a collar device. It seems to be about the same relative size as the officer hat badge. Also, collar devices come in pairs.
