That's why I asked if it could have happened at 29 Palms.

Could have been ripped any number of ways that do not include during his military service.

My thought is training accident due to the seriousness of the damage.

 

That being the small hole and cracked dent.

 

A near death experience so they kept the helmet and wrote the location, of said experience, on the cover.
