Being that it has Army side buttons and is an EM hat (i.e., center band is plain, not mohair braid), it's possible someone just added the PHS badge to an Army hat. However, you never know. If anyone wouldn't know the proper regs for uniforms, it would be a PHS Medical Officer. Any background on the cap? If straight from an attic or flea market junk box, I'd be more inclined to believe this is an "as worn" cap and not something put together by a collector.
