shirt size ?

That's what it is - smaller size for the shirt collar (as opposed to the larger size for the epaulet). These small size types were also commonly used on the overseas cap.

I'm assuming this is silver finish, not gilt? If gilt, it would be police.

world war I nerd

The shirt size rank and branch of service insignia was authorized sometime during the first half of 1918 (I forget which month). As mentioned above the diminutive insignia was intended for wear on the shirt collar when the service coat was not worn. The smaller versions of the rank insignia were also commonly worn on the left hand side of WW I officer's overseas caps.

Here's an example of a full size 'U.S.' collar device and a full size general's star (left) and a shirt sized "U.S." collar device next to a full sized general's star (right) as worn on the shirt collar. The inset shows the size difference between a full and a shirt sized officer's Motor Transport Corps collar devices.
